Collaborative partnerships improve access and coordination of care

Grandview viewed as the North Star among partners

During the Accreditation Canada survey, community partners shared their view of Grandview Kids as the “North Star” which guided best practices and innovation in paediatric rehabilitation. It is through meaningful and engaged partners that Grandview is able to commit to the strategic directions laid out in Building on Excellence.

Grandview Kids is an active member of Empowered Kids Ontario (EKO), the association for children’s treatment centres in the province. Together with EKO partners, Grandview works to promote investments, policies and programs based on best evidence and research to advance care at home, in schools and in communities.

Ontario is undergoing a transformation in its health care delivery, moving towards Ontario Health Teams (OHT) localized, groups of providers and organizations that are clinically and fiscally accountable for delivering a full and coordinated continuum of care to a defined regional population. Grandview Kids’ leadership is prominent in several key leadership committees within our local Durham Ontario Health Team (OHT-D) including serving as Chair of the OHT-D Performance Measurement Committee and within OHT-D Collaboration Council.

Grandview has a unique partnership with the Ethics team at Ontario Shores Centre for Mental Health Sciences

To provide our community with exemplary organizational and clinical support, Grandview has a unique partnership with the Ethics team at Ontario Shores Centre for Mental Health Sciences. This partnership involves Ontario Shores’ Ethicists serving as expert consults in the development of our own internal capacity and capability to address the evolving ethical decisions encountered by Grandview team. With new tools and resources to support our teams, we are confident that concerns raised by families are heard, discussed, and resolved together.

An emerging partnership with the Ajax Public Libraries has led to a Grandview Kids collection available on the Library website. This collection has been designed with input from clinicians, youth and families and provides resources in the form of books, videos, and virtual resources. The Ajax Public Library also made a generous donation with more than 200 books to Grandview’s Free to Read program, providing a new stock ready for eager readers when our Resource Library reopens.

Families continue to benefit from other partnerships, including those with the City of Pickering, the City of Oshawa, and the Municipality of Clarington that offer community-based recreation programs. Programs include “Let’s Get Active,” swimming lessons and summer camp programs.

Team Grandview continues to partner with agencies to provide ability acceptance presentations, expert advice on advisory committees, and soon to be the regional leader of a new Smart Start Hub. The Hub model will allow Grandview to build upon its existing partnerships with local educational, social and health care partners to ensure children, youth, and families receive early access to coordinated care. We are thankful for the opportunity to have such strong partnerships and look forward to watching our collaboration grow to benefit more families across the Durham Region.

Grandview isn’t just for Zoe, but is for the whole family.

 

Meet Zoe, a story told by Abby Valenciano

Zoe is three years old and every system of her body is affected by her rare genetic condition called CDG (Congenital Disorders of Glycosylation). She has low muscle tone, epilepsy, deformations, morphism, Cortical Visual Impairment (CVI), is globally delayed,
g-tube fed and non-verbal.

A rare medical diagnosis can be lonely and isolating. Zoe’s family has found the support they need at Grandview Kids. At Grandview Kids, Zoe receives physiotherapy, occupational and recreational therapy, and now participates in the Complex Care Program that allows accessible, local care.

Grandview Kids ensures parents are heard and advocated for in an empathetic and caring way. Zoe and her brothers are able to participate in accessible activities and events they would otherwise not be able to join. Zoe’s family has realized that Grandview Kids isn’t just for Zoe, but is for the whole family. Grandview Kids advocates for Zoe’s family and continues to be a vital source of encouragement, and ensures that every family member is considered as they continue to break barriers in the community.
